From Louisiana Archives ~ Cook, W H, Private Company A 4th Louisiana Infantry. Elisted May 25, 1861, Camp Moore, Louisiana. Present on all Rolls to December, 1861. Roll for January and February, 1862, Present. Very slightly wounded at Battle of Shiloh, April 6 and 7, 1862. Present on all Rolls from November, 1862, to August, 1864. Roll for August 31 to February 28, 1865, Absent. Wounded August 31st, 1864 in face.
